The problem

The hazard form gets signed in the truck at 6:55 am, copied from yesterday.

Then the trench is deeper, it rained all night, and the form says nothing about it. If someone gets hurt, that form is what the inspector reads.

The fix ↓

Great company to work with, incredibly detail oriented and easy to work with. We really appreciated how they focused on solving the problem, not just appeasing customer desires. The work was returned ahead of schedule with capabilities on par and beyond what we were expecting. …

    04 Questions

    Short answers.

What is a field level hazard assessment (FLHA)?

A short form a crew fills before a task. It lists the hazards, how you control them, and who was told. People call it an FLHA.

Is a field level hazard assessment required by law?

Safety laws require you to find and control hazards before work starts, but most do not name a form. Many construction companies and owners require a daily FLHA on their sites. Check your province or state and your contract.

How often should an FLHA be done?

Before each task, usually at the start of the shift. Redo it when anything changes: the task, the weather, the crew or the equipment.

What is the difference between a JHA and an FLHA?

They do the same job: list the hazards of a task and how you stay safe. In Canada the daily form is called an FLHA (field level hazard assessment). In the US it is called a JHA (job hazard analysis) or a JSA (job safety analysis). Pick Canada or United States in the tool and your form gets the right name.

Who should fill out and sign the FLHA?

The crew doing the work fills it in together, usually led by the foreman. Everyone on the task signs it.

Can AI fill out a hazard assessment?

Yes, as a first draft. Our AI Employee fills the form for each crew each morning, files it, and flags the ones that are missing. The crew still walks the spot and signs.
